Job summary

Unknown is seeking staff to provide routine nursing care to residents in accordance with each resident's assessment and care plan. You will identify residents before treatments, record accurate notes, and report any changes promptly.

Greet residents, assist with daily activities and meals, maintain room readiness, and ensure privacy and safety on every shift. You will also observe skin conditions and participate in nursing reports as required.

Responsibilities

• Identify residents before administering treatments using wristband or photo card.
• Record entries on flow sheets, notes, and charts in an informative manner.
• Report changes in resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Agree not to disclose resident's protected health information and report violations.
• Perform only nursing care procedures you have been trained to do.
• Ensure resident's room is ready for receiving the resident (bed made, name tags up, admission kit available).
• Greet residents and escort them to their room, introduce to roommate and others.
• Make resident comfortable and assist with daily functions (dental/mouth care, bathing, dressing).
• Check residents regularly if they cannot call for help and respond to calls promptly.
• Record resident food/fluid intake and report changes in eating habits.
• Keep water pitchers clean and filled and within reach.
• Perform after-meal care and continue to participate in nursing reports.
• Follow established policies and procedures, and cooperate with inter-departmental staff.
• Notify the facility if you will be late or absent, report exposures to blood/body fluids.
• Assist with lifting, turning, moving, and transporting residents as needed.
• Observe skin condition for decubitus ulcers and report injuries from unknown sources.
• Observe disoriented or comatose residents and report data as instructed.
• Perform special treatments as instructed.
