Job summary

PACS Group is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care and services to residents in accordance with care plans. You will perform resident rounds, assist with activities of daily living, and document care and observations under nurse supervision.

The role requires adherence to care plans, confidentiality, and timely communication with the care team. Strong interpersonal skills and ability to lift/move residents as trained are essential.

Qualifications

• Must possess a Certified Nursing Assistant license per state regulations.
• Ability to read and follow patient care plans.
• Commitment to maintaining patient confidentiality (HIPAA).
• Strong communication with residents, families, and staff.

Responsibilities

• Identify residents before care using ID methods.
• Record entries accurately in charts and flow sheets.
• Report changes in resident condition to Nurse Supervisor.
• Provide routine daily nursing care per care plan.
• Assist with daily activities and mobility as trained.
• Maintain a clean, safe, and comfortable resident environment.
• Respond promptly to resident calls and requests.
• Follow facility policies on blood/body fluid exposure and infection control.
