Responsibilities

• Identify residents before administering treatments or meals using wristband/photo ID.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and report any disclosures.
• Assist residents with daily functions and mobility; help with transfers, positioning, and transportation.
• Record vital signs as instructed and document food/fluid intake and meals.
