Job summary

Qualifications

• Maintain DFS certification.

Responsibilities

• Provide direct patient care as assigned in the patient's home.
• Assist with activities of daily living and personal hygiene.
• Support transfers, repositioning, and ambulation as needed.
• Record and report changes in the patient’s condition to the supervising team.
• Follow the Plan of Care and document tasks using EVV where required.
• Maintain patient confidentiality and provide compassionate companionship.
