Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide home care in the Wilson area, with a schedule Mon-Wed 12:00–17:00 and Fri 13:00–16:00. The role focuses on direct patient care in the client’s home, emphasizing dignity and comfort.

You will assist with hygiene, mobility, meals, light housekeeping and companionship, while following the Plan of Care and reporting changes to the supervising team. Travel within the assigned area is required.

Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Possess effective communication skills to include ability to read, write and follow directions.
• Ability to perform physical tasks necessary to treat clients who may include lifting, bending and standing.
• Must have sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene care (bath, shave, hair care, mouth care).
• Assist with transfers, repositioning and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by the supervising RN, PT, OT or ST.
• Prepare meals with client preferences and diets when directed.
• Provide companionship and activities customized to client needs.
• Light housekeeping duties as assigned.
• Participate as a team member to ensure high quality health care.
• Maintain complete and accurate records required by agency.
• Record and report changes in client's condition.
• Complete EVV tasks as needed and time sheets when required.
• Demonstrate dependability by punctuality.
• Adapt to client and family values.
• Assist in facilitating independence within client abilities.
• Perform other duties per the care plan.
• Call Action Health Staffing with questions or comments; uphold confidentiality.
• Notify immediately if abuse, neglect, unsafe living environments, or privacy violations.
