Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Strong reading/writing and ability to follow directions.
• Ability to perform physical tasks such as lifting and transfers.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene and activities of daily living.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care as instructed by supervising clinicians.
• Prepare meals considering client preferences and diets.
• Provide companionship and engage in activities.
• Light housekeeping as needed.
• Maintain and report changes per Plan of Care.
• Record tasks in EVV/time sheets as required.
• Uphold patient confidentiality and privacy.
