Qualifications

• Experience in home health scheduling or healthcare coordination preferred.
• Knowledge of EMR, scheduling software, and Microsoft Office.
• Excellent telephone, written, and verbal communication skills.
• Strong organizational skills and ability to multitask in a fast-paced environment.
• Maintain patient confidentiality and HIPAA compliance.

Responsibilities

• Coordinate and maintain daily and weekly schedules for field clinicians.
• Schedule patient visits, evaluations, start-of-care visits, recertifications, and routine visits.
• Monitor clinician availability, geographic assignments, and caseloads for coverage.
• Communicate daily schedules, patient assignments, and changes to clinicians and staff.
• Contact patients and caregivers to confirm appointments and communicate changes.
• Respond promptly to call-outs, cancellations, urgent scheduling needs, and unassigned visits.
• Track outstanding visits and help prevent missed or delayed visits.
• Coordinate with Clinical Director and office team on staffing and scheduling conflicts.
• Maintain scheduling records and update EMR and scheduling systems.
• Monitor visit frequencies and ensure compliance with physician orders and care plans.
• Assist with daily scheduling reports and productivity tracking.
