Job summary

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.

Responsibilities

• Interprets medication orders to ensure safety and proper use.
• Monitors therapy for significant drug interactions and optimizes outcomes.
• Communicates with physicians and nurses to adjust therapy as needed.
• Processes medication orders into the Electronic Health Record and maintains patient profiles.
• Oversees dispensing processes and ensures compliance with policies.
• Educates patients and staff about medications and participates in training.
