Qualifications

• Preferred training in specialty areas (neonatology, critical care, oncology).

Responsibilities

• Interprets medication orders to ensure safety and proper use; coordinates solving medication related problems with pharmacy, nursing and prescribers.
• Monitors therapy for drug interactions and therapeutic duplications; intervenes when needed.
• Evaluates medication choices and communicates issues and solutions to prescribers.
• Acts as consultant on pharmacokinetics and adverse reactions; participates in therapeutic drug monitoring.
• Documents clinical activities and interventions; maintains patient data in the EHR.
