Job summary

Cornell Scott - Hill Health Center is seeking a part‑time Clinical Pharmacist (20 hours) to join our patient‑centered care team in a collaborative FQHC setting. You will conduct MTM, manage chronic diseases, and partner with providers to optimize therapy while educating patients and staff.

The role supports medication safety and regulatory compliance and may involve dispensing duties as needed within the daily operations of the pharmacy.

Qualifications

• Degree from an accredited college of Pharmacy required. Residency preferred.
• Licensed or eligible to practice pharmacy in the state of Connecticut.
• ASHP or other national pharmacy organization membership preferred.
• Joint Commission standards experience preferred.
• Ability to communicate professionally with staff and patients regarding drug therapy.

Responsibilities

• Provide MTM and direct patient care for chronic diseases (e.g., diabetes, hypertension).
• Collaborate with providers and participate in population‑health initiatives.
• Educate patients and staff on safe, effective medication use.
• Support dispensing and daily pharmacy operations as needed.
• Contribute to medication safety and regulatory compliance.
