Job summary

Under the direction of the Family Health Centers’ Chief Pharmacy Officer, provides advanced pharmaceutical care to FHC patients as a key member of an interdisciplinary care team. Responsible for providing direct patient care, medication therapy management (MTM), and chronic disease management.

Responsible for ensuring compliance with federal, state, and local regulations and supports pharmacy operations as needed, while engaging in ongoing professional development and public health initiatives.

Qualifications

• Doctorate degree in Pharmacy is required.
• Active license to practice Pharmacy in Kentucky with clean record for Medicaid/Medicare payment.
• Immunization administration certification.
• Naloxone certification.

Responsibilities

• Provides advanced pharmaceutical care to patients as part of an interdisciplinary care team.
• Delivers direct patient care, medication therapy management (MTM), and chronic disease management.
• Ensures compliance with federal, state, and local regulations.
• Participates in interdisciplinary team huddles and case conferences.
• Maintains accurate documentation in the EHR and supports 340B drug pricing program audits and reporting.
