Cornell Scott-Hill Health Center in New Haven, CT, seeks a Part-Time Clinical Pharmacist to join our patient-centered primary care team. You will provide MTM, chronic disease management, and collaborative medication therapy within an FQHC setting, supporting safety, efficacy, and daily pharmacy operations.
The role requires CT licensure or eligibility, residency preferred, and active participation in national pharmacy organizations or Joint Commission standards familiarity is desirable.
We are seeking a Part Time ((20 hours) Clinical Pharmacist to join our dynamic, patient-centered care team. In this role, the Clinical Pharmacist provides comprehensive pharmacy services within a collaborative, team-based FQHC environment. Core responsibilities include chronic disease management, Medication Therapy Management (MTM), provider collaboration, population-health-driven initiatives, and education for both patients and staff. The Clinical Pharmacist also participates in interdisciplinary care team activities, supports medication safety and regulatory compliance, and contributes to dispensing and daily pharmacy operations as needed.
