Job summary

Cornell Scott-Hill Health Center in New Haven, CT, seeks a Part-Time Clinical Pharmacist to join our patient-centered primary care team. You will provide MTM, chronic disease management, and collaborative medication therapy within an FQHC setting, supporting safety, efficacy, and daily pharmacy operations.

The role requires CT licensure or eligibility, residency preferred, and active participation in national pharmacy organizations or Joint Commission standards familiarity is desirable.

Qualifications

• Degree from an accredited college of Pharmacy required. Residency preferred.
• Licensed or eligible to practice pharmacy in Connecticut.
• Membership in ASHP or other national pharmacy org preferred.
• Experience with Joint Commission standards preferred.
• Strong communication with staff and patients regarding drug therapy.

Responsibilities

• Reviews and interprets medication orders for safety and guideline adherence.
• Provides MTM and chronic disease management to patients.
• Identifies and documents medication-related problems.
• Designs drug therapy monitoring plans based on evidence.
• Performs CLIA-waived testing as appropriate and compliant.
• Educates patients, caregivers, and community members on medication use.
