ADP, Inc. in New Haven, CT is seeking a Part Time Clinical Pharmacist (20 hours) to join a dynamic, patient-centered care team at a provider-based FQHC.

You will deliver MTM, chronic disease management, and collaborative care within an interdisciplinary team, contributing to medication safety and compliance. The role emphasizes education for patients and staff, with opportunities to support daily pharmacy operations as needed.

Qualificações

• Degree from an accredited college of Pharmacy required.
• Residency preferred.
• Licensed or eligible to practice pharmacy in Connecticut.
• ASHP or national pharmacy organization involvement preferred.
• Experience with Joint Commission standards preferred.
• Ability to communicate professionally with staff and patients.

Responsabilidades

• Review medication orders for safety and adherence to clinical guidelines.
• Provide MTM, chronic disease management, and medication optimization services.
• Identify and document medication-related problems per facility standards.
• Design and implement drug therapy monitoring plans.
• Perform CLIA-waived testing when appropriate and compliant.
• Educate patients, caregivers, and community on safe medication use.
• Support dispensing and daily pharmacy operations as needed.
