Universal Health Services in Nevada is seeking a part-time Case Manager to oversee the discharge planning process from admission to discharge. Collaborates with physicians, nurses and social workers to ensure appropriate planning and referrals.

The role requires RN with Nevada license, 5+ years leadership and BLS; you will assess high-risk patients and support facility leadership with education on discharge practices.

Qualifikationen

• RN license required in Nevada.
• Baccalaureate degree in nursing; masters preferred.
• Minimum five years of nursing and leadership experience.
• Current BLS certification.

Aufgaben

• Understands and communicates the department and Hospital mission; holds regular staff meetings and ensures attendees receive information.
• Responsible for performing assessments on Medicare, Medicaid and high-risk patients to determine discharge planning needs; documents assessments.
• Communicate with physician regarding discharge planning needs; assist to facilitate post-hospital care.
