Job summary

Universal Health Services, through its Northwest Specialty Hospital expansion, seeks a part-time Case Manager (24 hours/week) to oversee the hospital’s case management process from admission to discharge.

You will coordinate with physicians, nurses, social workers and therapists to ensure timely discharge planning, appropriate referrals and individualized care plans across the care continuum.

Qualifications

• Graduate from an accredited school of nursing; baccalaureate degree required; masters preferred.
• Current Nevada Registered Nurse License.
• Five+ years of nursing experience and at least five years of leadership experience.
• Current BLS certification.

Responsibilities

• Oversee the case management process from admission to discharge.
• Collaborate with physicians, nurses, social workers and therapists to plan and coordinate discharge.
• Provide consultation and education to facility leadership and staff on case management and discharge planning practices.
