Job summary

Northern Nevada Health System is seeking a Part-time Case Manager (24 hours per week) to oversee discharge planning and coordination of patient care from admission to discharge. You will collaborate with physicians, nurses, CNAs, social workers, and therapists to ensure timely and appropriate arrangements for post-hospital care.

The role emphasizes utilization review, care planning, and education for facility leadership and staff in a busy hospital setting in Nevada.

Qualifications

• Graduate from an accredited school of nursing and be licensed as a Nevada RN.
• Baccalaureate degree required; masters preferred.

Responsibilities

• Oversee the case management process from admission to discharge.
• Collaborate with physicians, nurses, social workers and therapy staff to ensure appropriate discharge planning.
• Provide consultation and education to facility leadership and staff on case management, utilization and discharge planning practices.
