Job summary

You will collaborate with physicians, nurses, social workers and other staff to assess needs, develop care plans, and provide guidance on utilization and discharge practices in a complex hospital setting.

Qualifications

• Nurse with BSN or higher and Nevada RN license.
• Minimum 5 years nursing experience plus 5 years leadership.
• BLS certification current.

Responsibilities

• Oversee the case management process from admission to discharge.
• Collaborate with healthcare team to ensure appropriate discharge planning.
• Provide consultation and education on case management practices to leadership, clinical and financial staff.
