Job summary

Resources for Human Development (RHD) in Raleigh, NC seeks a compassionate Case Manager for the Community Transitional Recovery Program (CTRP). The part-time role supports individuals with high behavioral health needs as they transition from crisis settings to community living, coordinating resources and services for recovery and independence.

You will work within a multidisciplinary team to develop individualized recovery plans, advocate for participants, and help them achieve meaningful goals

Qualifications

• Bachelor’s Degree in Human Services or a related field (required).
• At least 2 years of experience providing support in Behavioral Health, Addiction Recovery, or IDD services.
• Valid Driver’s License and ability to transport participants.
• Ability to complete electronic documentation accurately and timely.
• Ability to provide community-based services as needed.

Responsibilities

• Team-Based Planning & Collaboration within a multidisciplinary team.
• Community-Based Case Management in participants' homes and community settings.
• Resource Coordination & Advocacy to connect with services and supports.
• Recovery & Transition Support to help participants develop independent living skills.
• Documentation & Compliance to maintain records per requirements.
