Responsibilities

• Support Medicaid-related placements and complex case coordination.
• Organize referrals, forms, and case updates with accurate notes and next steps.
• Coordinate tours and maintain community information in HubSpot records.
• Support outreach, events, and communications with referral partners and families.
• Receive guidance on placement workflows and escalate when needed.
