Help older adults and families move through complicated care transitions
Care Connect helps families navigate senior living and care transitions. We are hiring for two part-time associate openings across our care navigation, operations, and sales recruiting tracks, with the goal of developing successful hires into long-term Care Coordinators.
This position is a strong fit for someone interested in social work, human services, healthcare administration, or operations who enjoys solving practical problems, speaking with people, and bringing order to complicated situations.
You will support the team with tour scheduling, documentation, referral updates, and facility research. As you develop the necessary skills, you will take on supervised Medicaid placement coordination, substantive phone follow-up, and more complex case assignments.
This role also includes outbound professional outreach, event participation, and basic marketing support. All associates contribute to both responsive client service and the growth of Care Connect.
Our target is to evaluate readiness for advancement into a Care Coordinator role around month nine. Advancement depends on demonstrated skills, performance, and business needs; it is not automatic or guaranteed.
Support Medicaid-related placements and complex case coordination
This work supports the placement process. It does not include independently determining Medicaid eligibility, providing benefits-planning advice, or guaranteeing coverage or admission.
Tour responsibilities initially focus on scheduling and follow-up. Broader participation may be introduced after training and approval.
Study or experience in social work, human services, psychology, healthcare administration, public health, business, or a related area is helpful but not required. Customer service, admissions, scheduling, and administrative experience are also valuable.
Students, recent graduates, and career changers are welcome; current college enrollment is not required. This is a nonclinical position and is not advertised as a clinical social work placement or licensure-supervision opportunity.
Associates begin with defined support responsibilities and receive additional authority as they demonstrate competence. You will receive guidance on communication, documentation, confidentiality, escalation, and Care Connect’s placement workflows.
Associates do not independently perform clinical assessments, determine financial or clinical eligibility, recommend placements without required advisor review, negotiate contracts, or guarantee outcomes.
