Job summary

Qualifications

Responsibilities

• Engage the patient in care optimization.
• Empower the patient with knowledge and access to support resources.
• Improve the patient and family experience of care.
• Participate in Patients Are First measures and Medical Home goals.
• Referrals tracking to internal and external resources.
• Contact relevant community agencies to increase knowledge of services for patients and families.
• Educate per protocol.
• Utilize culturally relevant and language appropriate materials for target populations.
• Coordinate wellness and chronic disease activities with the health care team.
