Job summary

Neighborcare Health in Seattle seeks a Community Health Navigator Lead – Panel Support to deliver social services navigation for high-need patients, coordinating with RN care management and external partners.

You will manage a caseload, assess needs, connect patients to housing, food, behavioral health, and benefits, and document activities in the EHR, ensuring timely, person-centered care aligned with Neighborcare’s mission.

Qualifications

  • High School diploma or GED or equivalent relevant experience in health care or social services.
  • Must show proof of Community Health Worker Core Competency Course Certificate or obtain within 9 months.
  • Must show proof of Community Health Worker Pediatric Course Certificate or obtain within the first 12 months.
  • Minimum of 1-2 years of community health outreach, patient navigation, or related experience.
  • Experience working with diverse populations and navigating social services.

Responsibilities

  • Maintain a rotating caseload of empaneled patients and provide navigation support.
  • Assess needs and help patients access housing, food, healthcare, and other services; coordinate with team.
  • Collaborate with RN Care Management and other care teams to maximize access to navigation services.
  • Document screenings, social histories, and navigation activities in the EHR.
  • Maintain accurate documentation of care coordination activities.
  • Refer high-risk patients to RN Care Management or external supports.
  • Train new team members and assist in developing workflows.
