Qualifications

Responsibilities

• Engage the patient in care optimization.

• Improve the patient and family experience of care.
• Track referrals to internal and external resources and coordinate care.
• Educate using culturally relevant and language appropriate materials.
• Assist in outreach for wellness and chronic disease activities with the health care team.
