Samaritan seeks a Nurse Practitioner for its Palliative Medicine Inpatient team in New Jersey. This full-time role collaborates with physicians, nurses, and care navigators to deliver patient- and family-centered care across hospital partners.

Requirements include a Master’s in Nursing and NJ RN/APN licensure; DEA and CDS licenses are required. Experience in palliative care, oncology, hospice, or geriatric care is preferred but not mandatory.

• DEA and CDS licenses required
• Experience in palliative care, oncology, hospice or geriatric care preferred but not required

Responsibilities

• Provides a health assessment and diagnoses/treats symptom management.
• Collaborates with patients, families, and attending physicians for care.
• Develops and maintains internal and external relationships with partners.
• Works with Care Navigators to coordinate care across Samaritan programs.
• Ensures continuity of inpatient to outpatient palliate care with the team.
• Participates in after-hours call rotation; 40-hour work week with some on-call and weekends.
• Supports transfer of patients to outpatient palliate care as needed
