Samaritan is seeking a diligent Nurse Practitioner for Palliative Medicine in New Jersey. This full-time role involves collaboration with patients, families, and physicians while delivering care in our community and specialty hospitals.

Qualifications

• Experience in palliative care, oncology, hospice or geriatric care is preferred.
• Consultations in partner hospital settings.

Responsibilities

• Provide health assessments and manage symptom treatments.
• Collaborate with families and attending physicians.
• Maintain relationships with internal and external partners.
• Work with Care Navigators for coordinated care.
• Provide continuity of care with Complex Care Coordinators.
• Full time M-F (40/week) with afterhours call rotation.
• Participate in on-call and weekend rotation.
