Job summary
US Career Partners is seeking a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at Jersey City Medical Center in New Brunswick, NJ. The role offers mentors, physician/APP backup, and above-market compensation with a Monday–Friday schedule and generous time off.
The successful candidate will hold NJ NP/PA licensure, DEA/ CDS, and 2+ years in palliative care, with certification preferred.
Qualifications
- Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
- DEA and CDS registration, or ability to obtain by start date.
- 2+ years of palliative care experience required.
- Palliative care certification preferred.
- Strong communication and teamwork across disciplines.
Responsibilities
- Provide hospital-based palliative care consultations and follow-up visits across the inpatient continuum.
- Manage complex symptoms in patients with advanced illness.
- Lead goals-of-care and advance care planning conversations with patients and families.
- Collaborate with interdisciplinary teams and integrate into hospital workflows.
- Complete timely documentation aligned with clinical, billing, and quality standards.
- Spend substantial time on discharge planning and transitions between settings.
