Job summary

US Career Partners is seeking a Nurse Practitioner or Physician Assistant for inpatient palliative care at Jersey City Medical Center in Somerville, NJ. The role requires an active NJ license and national board certification, with DEA/CDS registration or ability to obtain by start date.

You will provide comprehensive palliative care, lead goals-of-care conversations, and work within a strong interdisciplinary team.

Qualifications

• Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
• DEA + CDS registration, or ability to obtain by start date.

Responsibilities

• Provide hospital-based palliative care consultations and follow-up visits across inpatient settings.
• Manage complex symptoms for patients with advanced illness.
• Lead goals-of-care and advance care planning conversations with patients and families.
• Coordinate with interdisciplinary teams and hospital workflows to integrate palliative medicine practice.
• Document timely, accurate, and compliant clinical notes for billing and quality.
• Contribute to discharge planning and transitions across settings.
