Job summary

Samaritan Healthcare & Hospice in Mt Laurel, NJ, is seeking a Nurse Practitioner for its Palliative Medicine Inpatient team. This full-time, on-site role collaborates with physicians, families and care navigators to deliver compassionate, patient- and family-centered care.

The NP will perform health assessments, manage symptoms, and support goals of care conversations while coordinating with hospital partners. Requires Master’s in nursing and NJ RN/APN license; DEA/CDS credentials preferred.

Qualifications

• Master’s degree in nursing is required
• RN and APN license in NJ are required
• DEA and CDS licenses will be required

Responsibilities

• Provides health assessments and diagnoses for symptom management
• Collaborates with families, physicians and care teams to provide patient-centered care
• Develops and maintains relationships with internal and external partners
• Works with Care Navigators to coordinate care across Samaritan programs
• Transfers inpatient patients to outpatient palliat ive med team for continuity of care
• Full-time, weekdays with after-hours call rotation and weekend rotation
