Mercy Hospital Springfield - HUB is seeking a Full-Time Palliative Care and Hospice Coordinator (RN) to manage patient care across clinic, consult, inpatient hospice, and outpatient services. You will participate in rounds and interdisciplinary meetings, assess eligibility, and ensure compliance with Medicare/Medicaid rules and Joint Commission standards.

The role requires an RN license, BLS, and strong collaboration with diverse healthcare teams.

Qualificações

• RN license in the applicable state or compact licensure
• Basic Life Support certification or completion within 30 days of hire
• Experience in hospice or palliative care is preferred
• Knowledge of Medicare/Medicaid regulations and Joint Commission standards

Responsabilidades

• Coordinate patient care within the Hospice and Palliative Care program.
• Attend care management rounds and interdisciplinary meetings.
• Assess patient eligibility and conduct concurrent care reviews.
• Ensure service quality and compliance with regulations and standards.
