Mercy Home Health seeks an experienced nurse to guide patients in the later stages of illness, coordinating interdisciplinary care and end-of-life planning within the Mercy system.

The role emphasizes advocacy for patient and family wishes, collaboration with physicians, case managers, chaplains, and other professionals, and leadership of weekly care conferences to ensure continuity across home health, hospice, and long-term care settings.

Qualifications

• Bachelor Degree required.
• Graduate Degree (Masters) preferred.
• Minimum two years experience with palliative or end-of-life care.
• Current nursing license in the State of Ohio; BLS certification required.

Responsabilités

• Lead care coordination for interdisciplinary teams to manage palliative care plans.
• Advocate for patient and family preferences within the healthcare system.
• Coordinate care across home health, hospice, long-term care and other settings; document interventions.
