Mercy Hospital Springfield - HUB is seeking a dedicated Palliative Care and Hospice Coordinator to oversee patient care across the Hospice and Palliative Care program, including outpatient clinic, consult service, inpatient hospice unit, and ongoing reviews.

You will attend care management rounds, assess eligibility, ensure compliance with Medicare/Medicaid and Joint Commission standards, and support accreditation efforts.

Formación

• RN with current license in the state or compact licensure.
• Associate's degree in nursing required.
• BLS certification within 30 days of hire.

Responsabilidades

• Coordinate care across the Hospice and Palliative Care program.
• Attend care management rounds and interdisciplinary meetings.
• Assess patient eligibility and conduct concurrent care reviews.
• Ensure compliance with Medicare/Medicaid regulations and Joint Commission standards.
• Support accreditation activities and use Epic EMR effectively.
