Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient- and family-centered home care. The role emphasizes supporting families, educating caregivers, and collaborating with the care team to deliver comfort, peace, and dignity in patients' homes.

The position requires an active RN license in the state, at least two years of RN experience (hospice preferred), a valid driver’s license, and CPR/BCLS certification.
