Mercy Hospital Springfield seeks a Palliative Care and Hospice Coordinator to oversee patient care across outpatient clinic, consult service, inpatient hospice, and outpatient hospice within the Hospice and Palliative Care program.

Responsibilities include attending care rounds and interdisciplinary meetings, assessing eligibility, maintaining compliance with Medicare/Medicaid and Joint Commission standards, and proficient use of the Epic EHR to ensure high-quality, compassionate care.

Qualifications

• RN with Associate's degree and current license in hiring state.
• BLS certification or completion within 30 days of hire.
• Ability to assess patients and develop care plans with strong communication.

Responsabilités

• Coordinate patient care within Hospice and Palliative Care program.
• Attend care management rounds and interdisciplinary meetings.
• Assess patient eligibility and conduct concurrent care reviews.
• Maintain knowledge of Medicare/Medicaid regulations and Joint Commission standards.
• Use Epic EMR effectively to document and coordinate care.
