Job summary

TriHealth is seeking a Palliative Care Nurse Clinician to support patients with serious illness within an interdisciplinary team. The role emphasizes education, symptom management, and advance care planning.

The ideal candidate will have an Associate’s Degree or Nursing Diploma with 4–5 years of clinical nursing experience and RN licensure. CHPN certification is preferred or obtained within 2 years of hire, with local travel and mileage reimbursement.

Qualifications

• RN license required.
• CHPN certification preferred or obtained within 2 years of hire.
• 4–5 years of clinical nursing experience in acute or primary care settings.
• Strong interpersonal and written communication skills.

Responsibilities

• Collaborates with the palliative care team to provide clinical support and education.
• Document assessments, plans and outcomes in the medical record in a timely manner.
• Educates patients and families on goals of care, symptom management and advance care planning.
• Coordinate care transitions with care management staff and other disciplines.
• Mentor nursing staff and students in palliative medicine practices.
