Job summary

TriHealth seeks a Palliative Care Nurse Clinician to join an interdisciplinary team providing clinical support to patients with serious illness. You will educate patients and caregivers, coordinate care, manage symptoms and medications, and assist with advance care planning.

The role is Monday–Friday with no weekends or holidays, with local travel between clinic sites and mileage reimbursement. CHPN certification is preferred or obtained within 2 years of hire.

Qualifications

• Associate's Degree or Diploma in Nursing is required.
• 4–5 years clinical nursing experience in acute or primary care.
• Registered Nurse license is required.
• CHPN certification preferred or obtained within 2 years of hire.

Responsibilities

• Actively supports palliative medicine referrals by timely response and information gathering.
• Educates patients and families about illness, options, symptoms, and end-of-life planning.
• Documents assessments, interventions, plans and outcomes in the electronic medical record.
• Coordinates with care management and other disciplines to support transitions.
