Job summary

Central Ohio Primary Care Physicians,Inc is seeking a Palliative Care Intake RN to support the Comprehensive Care team by triaging referrals, assigning to clinical disciplines, and coordinating timely care with PCPs. The role handles patient or caregiver calls, documents care plans, and collaborates with teams to improve outcomes.

Qualified candidates have 5+ years of clinical and case management experience; preferred CCM or CHPN certifications.

Qualifications

• 5+ years of clinical and case management experience in hospital, home health/hospice, geriatrics, or managed care.
• CCM certification preferred; bachelor's degree in nursing or CHPN certification.
• Strong knowledge of HIPAA requirements and patient confidentiality.

Responsibilities

• Review the medical record upon referral to ensure appropriate level of care and services.
• Perform complete assessment of patient health status and identify gaps in care.
• Collaborate with care teams, patients, and caregivers to achieve care plan outcomes.
• Communicate with referring providers to clarify referral needs.
• Coordinate triage of patient or caregiver calls and timely care with PCP or Comprehensive Care provider.
• Document in the medical record and care management tool reflecting planning and evaluation.
• Manage Epic in-basket messages for timely patient response.
• Assist with other duties as assigned.
