Job summary

Mountain Ridge Hospital is seeking a Palliative Care Clinical Nurse Coordinator to lead the palliative program and coordinate care for patients with serious illness. You will work with physicians, nurses, social workers, chaplains, and hospice partners to optimize quality of life and guide goals-of-care discussions.

The role requires RN licensure in Colorado, BSN preferred, CHPN certification possible within the first year, and at least five years of nursing experience in hospice or palliative

Qualifications

• BSN strongly preferred; current CO RN license; CHPN certification can be obtained within first year.
• 5 years of nursing experience; hospice/palliative care experience required; oncology/ICU experience preferred.
• Must demonstrate leadership, prioritization and strong written and verbal communication.

Responsibilities

• Coordinate and provide clinical leadership for the Palliative Care Program.
• Facilitate goals‑of‑care discussions and advance care planning with patients and families.
• Collaborate with physicians and interdisciplinary team to develop individualized care plans.
• Lead spiritual care coordination and oversee the Mountain Ridge Volunteer Program.
• Coordinate mortality reporting and participate in quality improvement and program development.
