Job summary

Qualifications

• Active RN license in state(s) of practice (Illinois).
• APRN/APRN-FPA/APNP or other APRN license in state(s) of practice.
• Active national board certification in area of clinical practice.
• Active DEA registration prior to hire.
• Illinois Controlled Substance License prior to hire.
• BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire.

Responsibilities

• Assesses, diagnoses, and determines treatment plans appropriate for age and condition.
• Manages conditions based on evidence-based care and risk assessment.
• Provides health promotion and disease management counseling.
• Works with an interdisciplinary team and escalates when needed.
• Maintains EMR documentation and compliant coding/billing practices.
• Facilitates coordinated care and clear communication among care teams.
• Performs procedures within scope of practice where applicable.
• Participates in quality, safety, and peer review activities.
• Involves in education and onboarding of new staff and students.
• Maintains productivity and quality metrics to ensure timely patient care.
