Job summary

Advocate Health Care is seeking a skilled nurse practitioner or advanced practice registered nurse for the Illinois Masonic - Palliative Care team. The role requires an active RN license and APRN certification, with a Master’s in Nursing and board eligibility.

You will collaborate with an interdisciplinary team to provide comprehensive palliative care, documentation, and patient education. The position is full-time, onsite, with 8-hour shifts, call coverage, and weekend rotations, supporting

Qualifications

Active RN license in state of practice and APRN/APRN-FPA/APNP or other APRN license.
Active national board certification in clinical practice and populations served.
Active DEA registration prior to hire. Illinois Controlled Substances License may be required.
BLS and/or ACLS, PALS, NRP as required by clinical practice before or within 6 months of hire.

Responsibilities

Assess, diagnose, and manage treatment plans appropriate for patient age and condition.
Order, perform, and interpret diagnostic studies and prescribe therapies.
Provide health promotion, disease prevention, and patient education.
Collaborate with interdisciplinary team and coordinate care across settings.
Refer to physician or team members when needed.
Escalate emergent or specialized care as necessary.
Maintain accurate documentation in electronic medical record.
