Advocate Health Care’s Carolinas Palliative Care & Hospice Group seeks a full-time Nurse Practitioner to join an interdisciplinary inpatient palliative care team in Stanly, NC. You will deliver compassionate care alongside physicians, APPs, nurses, social workers, and chaplains in a 40-hour workweek with an on-call rotation.

Responsibilities include inpatient palliative care consultations, symptom management, and family support, with physician backup and a focus on coordinated care.

Qualifications

• Ability to work within an interdisciplinary inpatient palliative care team.
• Provide symptom management and goals-of-care discussions.
• Collaborate with physicians, APPs, nurses, social workers, and chaplains.

Responsibilities

• Collaborate with the interdisciplinary care team to support symptom management, goals-of-care discussions, and coordinated care planning.
• Work closely with physicians and APP partners, with supervising physician support available.
• Participate in the call rotation with physicians and APPs.
• Provide inpatient palliative care consultation and ongoing support for patients and families.
