Job summary

Samaritan is seeking a Nurse Practitioner to join our Palliative Medicine Outpatient team in Mt Laurel, NJ. This part-time role involves consultation and follow-up visits across home, facilities, and clinics, collaborating with families and primary care physicians to deliver patient-centered care.

The NP will assess, diagnose, and manage symptoms and goals of care, build relationships with patients and staff, and participate in afterhours call rotation.

Qualifications

• RN, APN license in NJ are required.
• DEA and CDS licenses are required.
• Palliative Certification within two years of hire.
• Experience in palliative care, oncology, heart failure, geriatric care, or hospice is preferred.

Responsibilities

• Provides a health assessment appropriate to the visit, diagnoses and treats symptoms and goals of care.
• Collaborates with patients, families, and attending physicians for coordinated care.
• Develops and maintains internal and external relationships with team and partners.
• Works with support staff to meet needs of internal and external partners.
• Part time M-F (16/week) with afterhours call rotation (telephone calls no visits).
