Job summary

Amedisys in Hackensack, NJ is seeking a Nurse Practitioner to deliver home-based palliative care in collaboration with physicians. You will assess patients, manage symptoms, order tests, and develop individualized care plans within regulatory guidelines.

You will schedule visits, coordinate with the health care team and referral sources, educate patients and families about disease trajectory, and ensure compliance with Medicare requirements.

Qualifications

• One (1+) year experience as a Nurse Practitioner or three (3+) years as RN in geriatric, hospice or palliative care.
• Master's Degree or higher degree in Nursing.
• Licensed Nurse Practitioner in the state where services are provided.
• Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Authority to practice and prescribe as a nurse practitioner in the state where services are provided if prescribing is permitted by state and federal law.
• Medicare billing privileges as a nurse practitioner.

Responsibilities

• Provides palliative care for residents in their home or clinical setting in collaboration with physicians and/or medical directors.
• Performs medically necessary assessments meeting Medicare Part B requirements with documentation.
• Schedules visits based on medical assessment and the patient care plan.
• Orders and reviews treatments, lab, X-rays, and diagnostic procedures as indicated by the plan.
• Reviews results with the physician and adjusts the plan; reschedules follow-up as needed.
• Collaborates with physicians and other health professionals to ensure optimal care delivery.
• Coordinates care with hospitals, facilities, and primary care providers.
• Promotes palliative education to stakeholders in the community.
