Job summary

Life Tech seeks a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile palliative care team serving East Hanover, NJ. The clinician will have autonomy, build meaningful patient relationships, and collaborate with physicians and care teams to deliver high-quality in-home care.

The role requires in-home assessments, care planning, symptom management, patient/family education, and accurate documentation.

Qualifications

• Active NP or PA license in the state of New Jersey.
• Certification in palliative care or relevant experience strongly preferred.
• Minimum of 2 years clinical experience in geriatrics, hospice, or chronic disease management.
• Strong communication and interpersonal skills.
• Ability to work independently and travel to patient locations.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide comprehensive palliative care to patients with chronic or life-limiting illnesses.
• Conduct in-home assessments, develop care plans, and manage symptoms.
• Collaborate with physicians, caregivers, and facility staff to ensure continuity of care.
• Educate patients and families on disease progression, treatment options, and end-of-life planning.
• Document visits and maintain accurate medical records in compliance with regulatory standards.
• Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.

Role Overview
