Qualifications

• Board Certified, with a valid NP or PA certification in the state of New Jersey.
• 1-2 years of primary care experience with geriatric patients; hospice experience preferred.
• Ability to work independently and provide high-quality patient-centered care.

Responsibilities

• Provide high-quality primary and palliative care to homebound patients in Cumberland County, NJ.
• Perform physical examinations and develop care plans.
• Communicate effectively with office staff and clinicians across platforms.
• Attend quarterly medical staff meetings and contribute to team-based care.
• Develop impressions, diagnoses, and treatment plans; adjust as needed.
• Order diagnostic tests when medically necessary and prescribe medications.
