Job summary

AlignMed Partners is seeking a full-time Palliative Care Advanced Practice Provider (NP or PA) for a float position between our Albuquerque centers. The base salary is USD 120,000–130,000 per year with an incentive program.

The role emphasizes evidence-based care, interdisciplinary collaboration, and ongoing professional education. The ideal candidate will hold NP or PA credentials, with geriatric experience preferred, and be licensed to practice in the state.

Qualifications

• Graduate from nursing program with NP path or PA path and applicable state licensure.
• Two+ years experience with adult or geriatric populations preferred.
• Demonstrated ability to work in post-acute/long-term care settings with interdisciplinary teams.

Responsibilities

• Assess health status, diagnoses, and develop treatment plans in collaboration with care teams.
• Provide health education to patients and families and ensure timely documentation.
• Support nursing staff development and clinical education priorities in collaboration with center leadership.
• Participate in center-based quality initiatives and reduce unnecessary hospital transfers.
• Adhere to coding, billing, and compliance policies; support AlignMed Partners initiatives.
