Job summary
AlignMed Partners is seeking a full-time Palliative Care Advanced Practice Provider (NP/PA) to float between centers in Albuquerque, NM. The role emphasizes pain and symptom management, family support, and collaboration with the primary care team to improve resident care.
A Master’s in Nursing and NP certification are required, with RN licensure in good standing. Base salary ranges from $120,000 to $130,000 per year, with an incentive compensation program and relocation benefits for eligible
Qualifications
- Graduate from an accredited School of Nursing
- Licensed RN
- Nurse Practitioner certification with MSN
- Older Adult or Family NP concentration preferred
Responsibilities
- Provide primary and urgent care to nursing center residents
- Collaborate with Center Leadership and medical team
- Focus on pain and symptom management, psychosocial support, and advance care planning
- Adhere to Coding and Billing Compliance Program
- Support nursing staff education and clinical initiatives
