Job summary

Hackensack Meridian Health seeks an Oncology Nurse Navigator to guide patients from pre-diagnosis through treatment and survivorship. You will collaborate with the multidisciplinary team to ensure timely access to care and coordinate referrals to support services.

Qualified candidates hold a BSN with at least two years of nursing experience, and must demonstrate strong communication and computer skills. This role partners with physicians, nurses and allied staff to optimize patient outcomes.

Qualifications

• BSN required for team members hired on or after March 2024.
• Minimum of two years of nursing experience.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and Google Suite.

Responsibilities

• Coordinates and navigates outpatient care for oncology patients from pre-diagnosis to survivorship/end of life.
• Maintains relationships with medical oncologists, radiation oncologists, surgeons and related staff.
• Communicates with physicians and office staff to outline navigation programs and resources.
• Tracks patient flow through diagnostics and treatment services; documents in medical records.
• Provides referrals to financial counseling, nutrition, social work, and community resources.
• Participates in tumor board preparation and survivorship care planning as applicable.
• Supports performance improvement and aligns with regulatory standards for patient care.
