Hospice- is seeking a Palliative Care Navigator to manage enrollment in the care management program, craft individualized care plans, and conduct regular follow-ups. The role focuses on ensuring continuity of care and achieving health goals for patients in a full-time, Monday–Friday schedule.

The navigator will coordinate with clinical teams, monitor progress, and address ongoing patient needs to support overall well-being and program participation in a dedicated palliative care setting.

Qualifications

• Active CMA certification or willingness to obtain.
• Experience in a clinical office setting, palliative & hospice care preferred.
• Computer skills are required.

Description

We are seeking a Palliative Care Navigator, with a focus on principal care management. This is a full-time (40 hour) position, Monday through Friday. The palliative care navigator is responsible for enrolling patients in the care management program, developing individualized care plans, conducting follow-ups to monitor progress and address needs, and overseeing patient engagement and program participation to ensure continuity of care and achievement of health goals.
