Job summary

Qualifications

• BLS certification preferred.

Position Summary:

The Patient Care Navigator II position is responsible to provide care coordination, admit patients in electronic medical record system, schedule patients, triage incoming calls/messages, and potentially travel with the provider to assist with home visits and/or patient need assessments.Assist Primary and Palliative Care Nurse when needed. Responsible for a variety of clerical and support duties including typing correspondence and documents, special projects, faxing, errands outside organizationaloffice and other clerical support as required. Cross-cover palliative care service lines as needed.

Position Qualifications:

• GED or High School Diploma; One-year certificate from college or technical school; or a minimum two years related experience and/or training; or equivalent combination of education and experience.
• Graduate from an accredited Medical Assistant Program (CMA) or registered Medical Assistant Program (RMA) and completion of a clinical externship. Medical Assistant Certification.
• Experience in primary care, geriatric care; hospice or palliative care preferred.
• Basic Life Support (BLS) certification preferred.
• Is self-directed with the ability to work with little supervision.
• Demonstrates the ability to work well with health care professionals and an interdisciplinary team.
• Ability to read and interpret documents, policies and procedures, procedural manuals, and operating instructions.
• Ability to write routine reports and correspondence.
• Professional oral and written communication skills with ability to speak effectively and interact with staff and community.
• Ability to interpret and apply understanding to carry out instructions furnished in written, oral, or diagram form.
• Knowledgeable of and proficient in a variety of computer programs including word processing and spreadsheets, includingbut not limited toMicrosoft Excel, Word, and Power Point.
• Ability to operate various office equipment including computer, copier, and facsimile machines.
